Guidelines:
Minimum 3 months required placing on Account/Services on Reserve

Maximum allowance: 2 times per calendar year without additional expense.

Individual and Bundled services billed at 50% of existing rate.

Bundles that are Disconnected to avoid 50% rate and charged only for individual service(s) will be charged Early Termination Fee and require equipment to be returned.

Fees - Beginning December 2009:

Account billed at 50% using VR code

Minimum 3 months; no maximum time period

$10.00 service order fee for account changes other than putting on Reserve

$10.00 service order fee for unbundling services

$199.00 Early Termination fees apply to unbundled accounts; customer required to return equipment

$10.00 service order fee for account using Reserve more than two times a year
